Registration how to do?

Application process for Madhya Pradesh Nursing Council Registration:

-Select application form for registration

-complete address, mail id, phone number
-transcript from college (training start date and end date)
-10th and 12th grade certificate
-copies of testimonials – 2 testimonials -you can take from principal, vice principal, professor details
-training period/internship certificate
-All year marksheets
-Provisional certificate
-Previous registration if any (if you apply post BSc regn)
-passport size photo in college uniform with white back ground
-all documents must be attested by principal
-Original document must be submitted in nursing council and verify them by council.
Fees payment-around 2000 INR-net banking

Online application can do process for Migration, duplicate certificate, renewal of registration,
Renewal of Registration:

• For registration renewal application form, please enter your online issued registration number and date of birth.
• The applicant received registration certificate through offline process will select no and apply for the registration

Documents Required For Registration Renewal:

1. Application form for renewal of registration-duly filled should be presented along with original registration certificate and with self attested copies
2. Original registration certificate
3. Passport size photo
4. Registration certificate front and back side
