✔ Notary attestation required (Same as passport attestation requirements) - Educational Certificates
✔ Diploma/Degree/Post BSc/MSc
✔ CGFNS filled form
✔ Course completion certificate
✔ Mark sheets
✔ New transcript (must be sent directly from the college to CGFNS) - Experience Certificates
✔ Scanned color copies of all past and current experience certificates
✔ If currently working, get an updated certificate with a recent date
✔ No need to send physical forms, just update the system with:
Supervisor’s Name, Email ID, and Phone Number from all employment places
- Nursing License Verification
✔ License verification form – Must be filled and submitted from all nursing councils where you are registered
✔ If expired, renew the license before applying
✔ If near expiry, renew it before submission
✔ Some nursing councils do not fill verification forms; in that case, obtain a Good Standing Certificate - Professional Gap Explanation
✔ If you have any career gaps, submit a formal explanation letter - 10th Grade Certificate
