4. Complete indian address of teh canddiate
5. HMC Registration certificate
6. self attested copy of passport (complete with blank pages) with stamp of immigration
7. Travel history on plain paper as per passport enteries on
8. mention the purpose of GSC is required with name of teh country.
9. Degree certificate with college name

2. Application form along with request on plain paper (Mention for which

5. Self-Attested copy of Passport (Complete with blank pages) with stamp of
   immigration
6. Self-attested two latest passport size photographs. (if offline application)
7. Details of travel history as per passport entries.

7. CME hours, if applicable( usually 20 hours required every 5 years)
