Application Process
- Gather all documents which is required for the process.
- There should not be 2-year experience gap to apply for the process. IF less than 2-year gap in practice you can still apply for process and must be completed within 2 year gap ending and should activate license also.
- Apply in MOHAP site for professional evaluation
- Make payment for data flow
- Attend Prometric/written/Oral exam based on your professional title(total 3 attempts only)
- Apply for Evaluation letter. Evaluation letter validity for 5 years.
- With evaluation letter, data flow report, resume and valid good standing -search for job and once you get job employer will activate MOH license for you.
- Every year your company will apply for license renewal.

For nurses who have account in federal department of nursing website , will get CGFNS form filled and others who have account in new MOHAP portal, no CGFNS form will be filled as the new evaluation letter comes with barcode.
